This is a Community Health Nursing Supervisor position in Health Access, Clinical Services Division at the Delray Beach Health Care Annex and responsible for the supervision of staff working in the HIV /TB clinics. Clinic staff works very closely with TB program staff for management of TB cases. Responsibilities include planning and directing staff’s work, approving leave, conducting performance appraisals and having the authority for hiring and taking disciplinary action or effectively recommending such actions. .
This position is designated as a supervisor position and the position is required to maintain confidential information in accordance with the DOH and Palm Beach County Health Department Information Security Policy, Protocols and Procedures. This position may view, update or release "Confidential " information. Confidential Information Sets: Human Resources and Clinical Information Data.
This position has access to Q-Flow, Florida Shots, Internet, email, HMS E.H.R., HMS Report Portal, People First, TRAIN, PRISM, TB, HIV, Emdeon, DOH Public Health Pharmacy ordering/inventory system, and other electronic health system as it relates to the position.
This position is responsible for implementation and maintenance of the State Pharmacy program at the local level using the DOH State PFS (Pharmacy Forms System). The PFS is an electronic database maintained by the Central Pharmacy which can be used to order, receive issue, distribute stock, reorder patient specific medications, and maintain a perpetual inventory for all medications used by the county health department. Pharmaceuticals include Nurse issuance, bulk orders and patient specific prescriptions.
Incumbent is expected to communicate (by phone, e-mail or in person) ina a courteous and/or professional manner at all times. Maintains a good rapport with staff and clients, a positive attitude, is flexible, approachable and accepts change readily as evidenced by lack of substantiated complaints/confrontations with staff and clients. Incumbent adheres to established Health Department policies and procedures, promotes the Department as a positive, caring community organization through courteous behavior toward all clients and co-workers.
Incumbent supports the policies of the PBCHD and works with others in an effort to accomplish the goals of the Department. Incumbent makes a reasonable effort to adhere to established saftey procedures and practices in the work area. The position works as part of the management team to support the unit and Department to accomplish goals within the strategic plan.
Ensure efficient flow of the TB and HIV clinics by assigning fair and appropriate workload to various staff while maintaining excellent patient care. Provide new staff with clinic and nursing orientation and facilitate student perceptorship. Participate in center management meetings to discuss changes and maintaining current knowledge within practice areas and disseminate new information to subordinate staff. Ensure case management is initiated and completed by assigned staff. Ensure that broken appointments are followed up by appropriate staff.
CASE MANAGEMENT AND COORDINATION OF PATIENT TREATMENT IN COLLABORATION WITH TB AND HIV
Work with staff to ensure that HMO/PPO conditions are met
Coordination and implementation of new programs such as Test and Treat (HIV), Antidote Project (TB)
Continuous liaison with TB and HIV State and Local program staff
Strives to reduce potential infection exposure by pre-identifying infectious cases comint to clinic
Identification and coordination of in-service and staff development as necessary to meet the TB state requirements for:
TB care coordination and transfer of interjurisdictional cases to appropriate Counties
Management of EDN referrals and refugee referrals
Electronic transmission of X-ray referrals to State consultant
Coordinates and assigns the EDN referrals from the TB program
Ensure appropriate interjurisdictional transfers are completed
Ensure that State care coordination modules are up to date
Facilitates the entry into care of TB supspects referred by the TB program staff
Assists in contact investigation with TB program staff
Assistance with coordination of referrals and authorizations from patient's insurance providers
Ensures linkage for pregnant HIV patients with County Perinatal Nurse
Assist with coordination of new Test and Treat program in collaboration with HIV program staff
Ensures that the HIV referral clerk is functioning at capacity and will assist when a problem occurs - Assist with access to HMO provider portals for patient referrals, and advocating for patient eligibility to be able to best serve the patients.
Ensure that patients meet eligibility requirements
Attends regular HIV coordination meetings
MANAGEMENT AND QI
Coordination and preparation for Local and State audits (TB, HIV and PFS)
Produce utilization and QI reports as requested
Ensure customer satisfaction by fostering a pleasant work environment and immediately investigating any complaints and reporting same to supervisor.
Assist in data collection.
Communicate regularly with supervisor and keep administration aware of daily operations
Attend Center Management meeting and community meetings as required
Chair HIV and TB QI meetings
Ensure appropriate and adequate supplies are in place for efficient and safe clinic operation
Provide appropriate level of supervision and complete performance appraisals on subordinate staff in a timely fashion, working with staff on develpment and improvement issues.
Participates in Quality Assurance activities and in planned educational sessions.
The incumbent will maintain a pharmacy document list indicating the physical location of all documentation (e.g., invoices, issue documents, transfer documents, et al) that might be required during an audit or inspection. This list shall also include the physical location of the most recent complete pharmaceutical inventory (generally the annual inventory done June30th) conducted in the CHD. Patient profile forms and hard copy prescriptions will also be kept on file locally. The medications that are distributed in clinic must be labeled appropriately and signed by a provider when applicable. In addition to using the PFS for ordering and inventory control, an internal data base (Access) will be utilized to re-order the patient specific medications on time. A perpetual inventory of pharmaceuticals will be kept up to date. As a clinic supervisor the incumbent will ensure that all nursing staff that has access to this pharmacy are trained appropriately and any error that occurs is reported immediately to the Center’s administration and to the State Pharmacy.
Incumbent will supervise the designated pharmacy senior LPN position that works in conjunction with nursing and provider staff.
Assist the Center’s Community Health Nursing Consultant with the QI reports, nursing productivity and other Ad hoc reports. May be asked to perform other related duties in the Center or for the Department of Health.
Must adhere to the agency approved nurses dress code.

KNOWLEDGE, SKILLS AND BILITIES, INCLUDING UTILZATION OF EQUIPMENT, REQUIRED FOR THE POSITION:

Data collection and assessment skills. LICENSURE, CERTIFICATION OR REGISTRATION REQUIREMENTS: This occupation requires licensure as a Registered Professional Nurse in accordance with Chapter 464, Florida Statutes or eligible to practice nursing in accordance with Florida Administrative Code 210-8.22 or 8.27 or Certification as a Physician’s Assistant in accordance with Chapter 458, Florida Statutes. This position requires a Bachelor of Nursing Degree with 3 years nursing experience or an Associate Degree with 4 years nursing experience.

Some evenings may run past 5pm and Wednesday's are until 6pm. Hours are flexed or saved as compensation time.
