Job Summary

Partnership for Children of Essex (PCE) is currently seeking Care Managers.

We are looking for exceptional individuals who are motivated and committed to providing outstanding service each and every day to children and families in Essex County. Key responsibilities include:

  • Facilitate Child and Family Teams
  • Provide face-to-face contacts with clients at or above minimum quantity standards
  • Provide telephone contacts with clients at or above minimum quantity standards
  • Provide crisis on-call coverage for PCE as needed (PCE provides 24/7 on-call crisis services for all PCE clients)
  • Provide crisis intervention services to children and families as needed. Provide initial crisis intervention and management with linkage to other professional services as needed
  • Coordinate and facilitate comprehensive assessments of strength and needs of children and families in their community
  • Facilitate the development of ISP’s (Individual Service Plans) for children and their families that allows and respects individual choice, and is outcome based
  • Coordinate Care Management across multi-systems
  • On and ongoing basis, assess the quality of all services being offered to clients
  • Maintain accurate, timely, and comprehensive records for each child and family
  • Develop transition plans for the child and their family into other services
  • Other duties as assigned by supervisor or higher level management personnel

Qualifications and Skills

  • BA in a related field, such as social work, counseling, psychology, criminal justice or special ed., and a minimum of one year related experience. MA in related field preferred.
  • Experience with community relations and resource development are preferred.
  • Bilingual in Spanish, Portuguese and/ or Creole is preferable
  • Prior experience in managing wrap-around services a plus
  • Must possess excellent writing skills
  • Must be computer literate
  • Knowledge of the Essex County community preferred
  • Must pass a criminal background check
  • Must possess a valid driver’s license
